The patients that will be serviced will be of all ages – newborn, pediatric, adolescent, adult and senior patients: skilled/non-skilled nursing services, infusion therapy, pediatric services and primary care services (prescribed by the primary care physicians for assessment services) will be provided. The agency will care for each and every patient with the utmost respect and care for all patients. The goal is to treat each patient as if he or she our family member. Divine Home Nursing, LLC will be a service agency that will provide medical/non-medical (skilled/non-skilled) care to patients in the home, which include but are not limited to infusion therapy, pediatric care, and primary care. The services are described below:
Skilled Nursing Medical Care
Skilled Nursing Medical Care would include services such as: wound care, urinary catheters, and administration of medication, checking vitals, monitoring a patient’s progress, cardiac rehabilitation, diabetic care, dietary regimen, colostomies, tube feeding, ventilator maintenance, evaluations, and teaching. The skilled nursing medical care is an extension of hospital-based nursing-in most cases. The nurse will also establish measurable goals for each patient and keep detailed notes and reports to document in the Electronic Medical Record (EMR).
Ukskilled Care
Unskilled Personal Care is care to patients that addresses activities of daily living. Such as: bathing, dressing, grooming, incontinence, shaving, dental care, eating, and bathroom assistance; and may include but not limited to proper nutrition, home management, housekeeping tasks. Ambulation, mobility positioning assistance is also included. Serving and warming meals may be included. Companionship Care (Sitter) Service is a service at provides transport and escort services, meal preparation and meal service. Keeping company with the senior client. Transportation to medical appointments, or short errands is also included, and travel companionship as well.
Infusion Therapy
Infusion Therapy is performed by a licensed RN through orders given in a prescription by a physician through a home infusion therapy pharmacy. Patients that receive infusion therapy are given medication intravenously. Medication is given through a needle in that vein, or catheter, instead of orally. These patients are in need of receiving the medication other than orally as in intramuscular injections and through epidural routes (into the membranes surrounding the spinal cord). The Infusion Therapy service will be performed on patients with cancers, congestive heart failure, Cohn’s Disease, hemophilia, immune deficiencies, multiple sclerosis, rheumatoid arthritis, and other patient in need of intravenous medication.
Pediatric Home Care
Pediatric home care will be given to ill children when it is medically necessary to have medical care but not necessarily have a need to be hospitalized. Sometimes the home health care is an extension of hospital care or continued care. The care for these children will mostly come under skilled/licensed pediatric nursing care. The pediatric nurse specializes in providing professional skilled nurses who can care for a medically fragile child of any age. The nursing care that the child receives will be of the highest quality just as if she would receive in the hospital. The pediatric nurse will be licensed and knowledge able about children with special needs such as: children that have a feeding tube, a tracheotomy, a ventilator, and a variety of monitoring equipment. The nurses will be prepared for both respiratory and cardiac emergencies in the home setting. The nurses will be also receiving CPR and /or PALS (Pediatric Advanced Life Support) certification. The pediatric nurse will be knowledgeable about growth and development and will teach parents and patients about their health through continuing education.
Primary Care
Primary Care is RN nursing care that is administered to patients that need a physical exam but do not have access to go to the physician; and to patients needing a physical for life insurance policies. The patient will have all vitals taken – Blood Pressure, temperature, weight, oxygen levels taken, EKG will be taken and recorded. All services will be taken a licensed professional according to the prescribed orders of the physician according to the state regulations of a PHCP.
Women’s Health Care
Care for new mothers before and after the delivery of the baby. The service may include skilled or unskilled nursing care. Personal plans and training in baby care. Pregnancy complications, premature births, multiple birth, feeding or lactation assistance, and postpartum care and assistance are provided. The skilled nursing care will be under the prescription of the physician, whereas the unskilled care will be more in line with Companion Care and/or Personal Care services.
